Registration Form Request

**You will need to fill out one registration per child.**
THE REGISTRATION is now Closed for the 2024 Fall Soccer League.
Your child must be 4 years old by October 1, 2025 to register.
You will need to provide a completed Code of Conduct and Liability Waiver for your child prior to collecting your uniform.
